How to Fill the MSP Feedback Form
In order to activate Mesh for a new Managed Service Provider (MSP), a Customer Feedback Form must be completed. This form ensures that all necessary details are recorded and routed correctly to process your request. Below is a detailed guide on how to fill in each field.

Contact Details
These fields identify the requesting MSP and provide the necessary contact information.
- Full Name
Enter the partner’s company name. Use the full official business name registered in the partner system.
Provide the primary contact email address for your company.
- Phone Number
Include a valid phone number.
- Country
Select the MSP’s registered country of operation from the drop-down menu.
Feedback Details
This section categorises and clarifies the nature of your request.
- Category
Choose the appropriate classification. For new feature needs, select Feature request to ensure proper routing to product and sales teams.
- Feedback for
Select the relevant area or process. For a purchase request, choose Purchase.
- Comments
Provide an explanation of your request or add any comments you might have.
Overall Rating
Selecting an overall rating is very important, as it helps us understand your experience. The following choices are available:
Poor — The current process, service, or product did not meet expectations, with significant issues or gaps that prevented effective use.
